Prescription Refill Policy

We offer patient prescription refills at every office visit.  We suggest that you bring a list of your current medications.  Should you require a refill outside of a visit to the office, please give at least a 72-hour advance notice for the request.

It is our policy that all patients on prescription medications must be seen on a regular basis as determined by their care giver.  We may decline a request for prescription refill if a patient fails to keep follow up appointments on a persistent basis.

You will speak with a receptionist who will ask:
Your name
Date of birth
Phone number
Physician’s name
Medication name
Dose
Frequency
Quantity

Please indicate the method of processing you prefer, i.e. fax the prescription to the pharmacy, mail-order pharmacy, mail prescription form to patient, patient will pick up, etc.  In many cases, renewals can be sent to the pharmacy electronically.
REMINDER: Prescriptions for controlled substances MUST be picked up.  Due to state law, these prescriptions cannot be called in or sent in electronically.

Please be advised that when you request that prescription be mailed, delays may occur due to factors we cannot control such as delay in postal service.  If you or your pharmacy has not received your prescription within in 72 hours, please call the appropriate office and ask to speak with a nurse regarding the status.
